The enrollment form must be completed by the patient or guardian. The amount of time will vary to complete additional steps in the process (such as, requesting authorization from the health plan, resolving coverage issues, locating a DME provider or pharmacy, etc.). The entire process could take a few business days or several weeks. Formula4Success is here to assist you throughout the process, and you and your health care provider will receive an initial enrollment notification followed by weekly status updates.
