Formula4Success will research your current insurance to see if our prescribed product is covered by your health plan, we will explain your benefits and rights to coverage, and we will help locate an in-network provider to render service. Formula4Success will work together with you and your health care provider to help improve your access to formula and insurance coverage.
The first step is to complete the Formula4Success enrollment form and submit it via online, email, fax or mail. If you have access to your prescription and letter of medical necessity then you can submit it along with your enrollment form. If you don’t have access to that information then don’t worry, Formula4Success will contact your health care provider and request to obtain your Vitaflo prescription and letter of medical necessity.
Formula4Success provides caring, personal attention to each patient. The first step in the process is to verify your health plan benefits which typically takes 2 business days. Time to resolve additional steps in the process (such as requesting authorization from the health plan, locating a supplier, resolving coverage issues, etc) will vary from case to case and could range from a few business days to several weeks. An initial enrollment confirmation email will be sent to the healthcare provider and patient/guardian, followed by a weekly status update.
Formula4Success will investigate your specific health plan to see if coverage is still possible, because a previous denial does not necessarily mean you do not have coverage.
